- PROMISSORY NOTE & INSTALLMENT PLAN AGREEMENT FORM: This agreement is used if the defendant/indemnitor have made a payment agreement with their Montana bondsman.
- TRUST DEED & NOTE FORM: This is a standard agreement form which includes the rules of the bail bonds contract. This form is signed by both the defendant and the indemnitor.
- COLLATERAL RECEIPT FORM: This is a promise to pay the full amount of the bond in the event the defendant breaches the bond.

- Once completed you’ll:
- Bring the forms to your bail meeting with Lisa’s Family Bail Bonds, or
- Print, sign and mail the forms to our bail office in Billings, MT
